Supporting Youth with Developmental Disabilities Through Life Transitions
E3 Community Services offers Future Transitions, a support service designed to help youth and young adults (ages 13 to 25) with developmental disabilities transition from school to adulthood. This service aims to provide guidance, resources, and planning to support people as they navigate the shift to post-secondary education, employment, or community living.
The Future Transitions program provides access to a dedicated Transition Coordinator who works closely with families, schools, and community agencies. This role is essential in helping young people explore their interests, skills, and goals, ensuring that their transition plan is personalized and comprehensive.
